Abstract: Clinical perspective on anticoagulation with DOACs in kidney disease
The use of direct oral anticoagulants (DOACs) in patients with chronic kidney disease (CKD) presents unique clinical challenges due to altered pharmacokinetics, limited evidence, and varying recommendations across guidelines. This presentation will provide a critical appraisal of current literature evaluating the safety and efficacy of DOACs in CKD, including the dialysis populations. It will further analyze and compare the pharmacologic and pharmacokinetic characteristics of each DOAC, with emphasis on kidney clearance, dose adjustments, and therapeutic considerations relevant to kidney function. Finally, the session will highlight knowledge gaps and uncertainties that persist in this area, underscoring limitations in existing studies and opportunities for future research to better inform clinical decision‑making. Together, these elements will equip clinicians with a deeper understanding of DOAC use in CKD and support more informed and individualized anticoagulation management.
