Abstract
Antiphospholipid antibody syndrome (APS) presents unique diagnostic and therapeutic challenges due to the frequent interference of antiphospholipid antibodies—particularly lupus anticoagulants—with phospholipid‑dependent coagulation assays. This presentation provides an overview of APS classification criteria and reviews the laboratory methodologies used to monitor anticoagulant therapies in affected patients. Participants will learn to identify the limitations of clot-based assays such as the aPTT and PT/INR in APS, understand the value of chromogenic anti‑Xa and chromogenic factor X testing, and apply appropriate monitoring strategies across heparin anticoagulants, vitamin K antagonists, direct oral anticoagulants, and direct thrombin inhibitors. Case-based scenarios will illustrate common and complex clinical situations, highlighting practical approaches to mitigating assay interference and optimizing therapeutic decision-making. By the conclusion of this session, attendees will be able to select and interpret anticoagulation monitoring tests accurately in patients with APS to enhance safety and treatment efficacy.
